Service Summary
The Out of Hours Nursing Service provides nursing care for patients of the 7 Integrated Community Care Teams across Western Cheshire. The service operates from 17.15 – 08.30 seven days per week and works closely with the GP Out of Hours Service. Visits are mainly unplanned, primarily to support palliative and end of life patients, and their families or carers, for symptom control, syringe driver management, psychological support, verification of death and bereavement support. The team also visit patients with a range of clinical needs including pressure ulcer management, wound care, male, female and supra pubic catheters, PEG medication and feeds. Short term support is provided for patients whose carer may be unwell or has a crisis.
How to Refer?
Either by phone or on EMIS. The answerphone is checked regularly whenever the care co-ordinator is not on duty. Referrals are received from Community Care Teams, Single Point of Access, Crisis and Reablement team, Rapid Response, GP Out of Hours, Families and carers, Acute care and Care Homes
