Service Summary
We are a group of health and social care professionals who work with local GP practices to support you at home, providing health and social care
The team include nurses, therapists and social workers
We also liaise with other local authorities and voluntary agencies.
The service is delivered jointly by Cheshire and Wirral Partnership NHS Foundation Trust and Cheshire West and Chester Council.
How to Refer?
Referrals to the teams can be made by GP practices, hospitals, hospices and other agencies. Patients, family members and carers can also refer in to the service by contacting the relevant Care Community team by phone.
