Summary of Service
The multi-disciplinary Community Stroke service provides care to patients following a stroke but also to support early discharge from hospital and rehabilitation in the community.
The team is formed of Physiotherapists, Occupational Therapists, Speech & Language Therapists, Dieticians, Nurse, social workers and a Cognitive Behavioural Therapist.
The team work closely with the stroke association to provide ongoing support and care after a Stroke.
How to Refer?
The service is only available through Therapy/Consultant/GP/Stroke Association referral.
