Service Summary
We provide advice and support to patients in the community who have been diagnosed with cancer or other life limiting conditions with the focus on improving quality of life. We monitor individual patients to effectively control their disease related symptoms, which may be physical, psychological or spiritual.
How to Refer?
Referrals are made using a multiagency referral form. This form is in the process of review by the Hospice of the Good Shepherd. The current form is enclosed which has incorrect telephone numbers. However, there is a BT divert which connects callers directly to the Team.
