Service Summary
The CCICP Wheelchair service based at Congleton War Memorial Hospital, provides Specialist Assessment against eligibility criteria for the provision of basic wheeled mobility and postural equipment. The service provides care to patients living in the Vale Royal, East and South of Cheshire.
How to Refer?
Initial Referrals are received from GPs, Consultants, Physiotherapists, Occupational Therapists, Community Matrons, District Nurses, Specialist Nurses, Practice Nurses
